Travel all the way back to the Church in the southeast quadrant of the map. Walk around to the left exterior wall. You will spot a convenient stack of wooden boxes. Jump up these crates to mantle your way onto the church roof. One final bell is waiting for you up there. Interact with it to permanently complete the secret. The game hands over the loot instantly, meaning you do not even need to extract to keep it.

All that trekking around the desert rewards you with the Bell Shot Joker. This loadout card is an absolute game-changer, especially during the early hours of your playthrough when healing is scarce.
